I think if you're playing free for all, the first person to 576 takes first but has to wait until QLs are in before progressing. Second is first person to 216. No jumping these spots once in. Can add points once QLs are in and start topping up.

IMHO, rules are up to the host.
In general, for free style launches with more than 3 QL's participating, only 576 rule should apply (which is universal for any launches). The rest should be first come first serve and use common sense. Otherwise it will delay the launch time and take the fun away.
You can set any rule for your own organized launches though.
